IMPROVING SAFETY IN HEALTHCARE AND SERVICES<\/b><\/p>\n

Everyone’s Business<\/b><\/p>\n

Whether you are a patient, a loved one, or a visitor, if you witness a risky situation or an accident, it is very important that you inform the staff immediately. They will take the appropriate measures, including reporting an incident or accident.<\/p>\n

A risky situation can include, for example, a risk of falling that you identify by seeing a wet floor, a crowded hallway with many patients, defective equipment, etc.<\/p>\n

In fact, your collaboration is essential in what our institution calls “risk management” and improving safety in the delivery of health care and services.<\/p>\n

Risk Management<\/b><\/p>\n

At Vita M\u00e9dicale<\/b>, analyzing incident and accident reports allows us to continuously improve the care and services provided to the public. This analysis allows us to implement concrete measures to prevent further incidents or accidents.<\/p>\n

Reporting an Incident or Accident<\/b><\/p>\n

Reporting an incident or accident is an obligation under the Act respecting health services and social services. This law aims to promote user safety and, consequently, the continuous improvement of the quality of care and services provided.<\/p>\n

What is an incident?
<\/b>A risky situation that may present a danger, but does not affect the user (examples: water damage, a broken window, etc.).<\/p>\n

What is an accident?
<\/b>A situation that directly affects the user, with or without consequences for them (examples: medication error, user fall, etc.).<\/p>\n

Disclosure of an Accident to the User<\/b><\/p>\n

Following an accident, whether or not the user is aware of it, a disclosure may be made. This notification is provided verbally to the user or their loved ones and contains all information related to the accident as well as the preventive measures taken to prevent its recurrence.<\/p>\n
